Application Process | Please fill out an application form or call to arrange a meeting to fill out a form with the Executive Director. You must have a face to face meeting with the Executive Director when you are filling out the police background check form. You must provide 2 forms of official id (i.e. drivers licence, medicare card, passport …) at the meeting. |
